I have tried every variation of this possible and I'm stuck. What is the correct format to get the proper output?

I am unable to get the output. I have tried uppercasing both strings in the same line and tried the + operator without any luck.

var id = "23188xtr";
var lastName = "Smith";

var userName = id.toUpperCase();
username+= '#' + lastName.toUpperCase();

<html>
<head>

Your current code is actually ok, you just have a typo. You have spelled the userName variable as username on the last line. And since JavaScript is case-sensitive that is an issue.

If you correct that typo your code will work. Though it can be shortened by combining the last two lines into one like this:

var id = "23188xtr";
var lastName = "Smith";

var userName = id.toUpperCase() + '#' + lastName.toUpperCase();
