In Treehouse's project files for “Adding a Contact Form”, a Problems During Load message displays instead of header.php

Please advise on suggestions regarding the following unexpected outcome during the "Adding a Contact Form" stage for "Building a Simple PHP Application" (stage 4 of 7).

Background - In Treehouse's project files for “Adding a Contact Form”, a 'Problems During Load' message displays instead of header.php

Outcome - When header.php is loaded, the following is displayed:

Shirts 4 Mike • ">Shirts • ">Contact • Shopping Cart

Other notes - I have confirmed php is installed and functional on my machine, including for other .php files. Also, I tested the header.php again with my code, and re-tested with all Workspaces files and Download/Project files provided throughout Treehouse videos for this section.

Please also advise if any related suggestions regarding the contact.php file.
