.index.htm issue in analytics

I've noticed that when I look at the pages people have visited in Google analytics I'm seeing www.masteel.co.uk.index.htm instead of www.masteel.co.uk or www.masteel.co.uk/index.htm

Are visitors attempting to enter the site through the .index.htm address or is this just a blip in GA.

Do you have any special filters set up in GA that would alter the way in which things are reported? My understand with GA is that by default, the data in your reports includes only the Request URI and not the domain name, but with filters you can change that.
