You have a couple little things going on.
1) You didn't properly close your input tag. This is how the input tag should look:
<input type="text" id="name" name="user_name" /></input>
2) The opening text area tag is missing it's ending bracket ">". This is how it should look:
<textarea id="comment" name="user_comment"/></textarea>
3) You don't want quotes around the button text. This is how it should look:
<button>Submit comment</button>
4) The opening bracket of the closing form tag was chopped off in your code. It should look like this:
</form>
